Transforaminal Lumbar Approach for Interbody Fusion in Treatment of Degenerative Disc Disease and Low Grade Spondylolisthesis,AHMED SAYED A. MONIEM
Abstract
Transforaminal lumbar interbody fusion is one of the techniques which are used in interbody fusion and posterior decompression at the same time. Approach to the disc is done through posterior Transforaminal route which enables the removal of the intervertebral disc totally with preserving the nerve root by being laterally to it.
Objective: Evaluation of surgical technique, indications, clinical outcome and complications of TLIF.
Material and Methods: Retrospective study of 10 patients with degenerative disc disease and low grade spondylolisthesis operated upon in Kasr Eleiny Hospitals and Emirate Zayed Hospital from 2009 to 2011. Patients were 3 Females and 7 Males. Range of age was between 45 and 60 years, mean age was 50.8 years. Clinical presentation included mechanical back pain ranged (100%) and radicular symptoms (50%) with no motor weakness. Exclusion criteria included pathological conditions of the lumbar spine (infection, trauma, tumors).
Results: TLIF was done in single level in 8 cases and 2 levels in 2cases. Posterior transpedicular screws, rods and cages were used in all patients. Back pain was improved in 90% of cases and the radicular symptoms were relieved in 80% of cases. Radiologically postoperative CT and X-rays of the lumbar spine showed proper fusion of the operated level. Follow-up of the patients radiologically and clinically for 22 month. Postoperative complications noticed in 2 cases in the form of poor improvement of back pain postoperatively and mild radiculitis.
Conclusion: TLIF is a technique which offers a simple, safe and effective treatment for adult degenerative spondy-lolithesis with great improvement of life quality of cases and surgery satisfaction.
